[UPHPU] Associating data with radio buttons

I have a form that has 26 questions that have true false radio buttons associated with each of them. I am able to save the data created as a new record but I can not figure out how to associate the data with the radio buttons to display and edit the data.

I've searched Goggle without luck. Has anyone out there been able to do this?
